Inaugural leader of a 7 town collective impact partnership working together to improve the health and quality of life for Connecticut’s Lower Naugatuck Valley residents. Identify opportunities and create solutions to increase coordinaton and collaboration within the 40+ partnership organizations and with diverse stakeholders in the Valley region. Key Accomplishments:• Develop a community hub referral system linking partnership members and connecting community members with resources addressing social drivers of health and barriers to equity. The system is comprised of care coordination, an electronic referral platform linking partnership members and a staffed navigation referral line. • Lead the triennial community health needs assessment for the partnership, local hospital, community foundation and regional health department incorporating community engagement throughout the process. Secure $250K funding for the assessment. • Design and oversee the partnership organizational structure including executive committee development, strategic plan, staffing, budget, strategy restreats, by-laws, surveys and membership development.• Build an on-going community input and communication strategy to ensure community voice guides the partnership strategic plan. • Represent the partnership in the community and establish strong collaborative relationships with community leaders, local government and other regional stakeholders.• Manage the $600K partnership fund and increase funding through growth and sustainablity plan.
