    Provide intensive community-based mental health services for children and adolescents experiencing emotional or behavioral challenges to prevent out-of-home placement and stabilize family systems. Deliver individualized, trauma-informed, and strengths-based mental health interventions while coordinating care across multidisciplinary systems (schools, courts, and community supports).Clinician must maintain an active and unrestricted registration as a Qualified Mental Health Practitioner-Trainee (QMHP-T) through the Virginia Board of Counseling.Key Responsibilities:* Develop individualized treatment plans, interventions, risk assessments, and safety plans in collaboration with the client (child or adolescent) and their caregivers/families.* Plan and deliver regular counseling sessions and behavioral interventions in-home and in community settings, applying evidence-based behavioral and family-systems approaches to assist clients in working towards their treatment plan goals and objectives.* Participate in Care Coordination as an integral part of overall client treatment; collaborate with case managers, healthcare providers and educators to support holistic treatment and family stabilization.*Coordinate, managing and attend care coordination appointments (e.g, IEPs, FAPT, Family Partnership, and court-related meetings) to ensure wraparound service delivery.* Provide crisis intervention, on-call support, and safety planning coordination with emergency and psychiatric services when necessary. Must maintain updated CPR/First Aid certifications.* Monitor comprehensive intake and psychosocial assessments to determine clinical needs, risk factors, and functional impairments.* Maintain ethical and timely documentation, including progress notes, treatment plans, reports, incident documentation, and discharge summaries. Uphold HIPAA and professional ethical standards in all clinical and administrative work.
