   It is the vision of Hearts N Hand to be the educational face of transplant success in the United States and abroad by integrating physical activity, healthy curriculums, youth developmental programing and in and out patient advocacy and follow-up.The organization will create education of healthy medial, dietary, physical and emotional lifestyle choices for pre-transplant candidate, recipients, donor’s; as well as the community at large.Through networking, social activities, information sharing and relationship building; the transplant community (both consumers and service providers) will grow in their knowledge and trust of Hearts N Hand as a caring and competent free resource. We offer client centered supportive services for both pre- and post- transplant patients, beyond and in support of traditional health care currently available. As part of our physical education and youth development component of our organization we have created the Hoopin’ Hearts semi-professional basketball team. In patient curriculum will consist of but not limited to-Compliance and non-compliance-Bridge to transplant-Guidelines for Use of MCS-Destination TherapyOur case Management component will provide 1-on-1 service planning and advocacy.Patients having compliance issues will result in an unsuccessful discharge from the program. For unrelated issues such as decrease in numbers and/or possible rejection, patient will be followed up to 6 months after first annual. Hearts N Hand will also provide family support outside of medical care. We also will stress the importance of organ awareness and the ever growing need for organ donation; particularly in urban and rural communities. Client referrals can be made by doctors, co-workers, family, friends or self-referrals. The process is as follows:1. Patient is referred2. Patient assessment and consultation3.
