Medical Social Worker II at WellBound San Mateo, San Carlos Home and San Carlos In Center Kaiser PatientsWorking in an interdisciplinary setting with Nephrologists, Nurses, Dieticians, Dialysis Techs and Medical Assistants, I use my expertise and clinical knowledge as a Medical Social Worker to help guide, educate and support patients across all areas including- Best options for treatment, health and wellness when living with kidney disease- Pathways to enhancing quality of life- Recruiting a living donor for transplantAs the medical social worker, I\'m responsible for supporting the social and emotional well-being of the patients assigned to my care. I conduct psychosocial assessments, participate in interdisciplinary care planning and team review of the patient\'s current psychosocial needs (which in the home dialysis setting happens on a monthly basis with the patients actively engaged), and I provide care management services to patients to assist them in coping with issues associated with chronic kidney disease and dialysis treatment (especially home dialysis, my true area of expertise).Initially worked strictly at WellBound San Mateo with a caseload of 80-100 patientsAs of July 2024, working at 2 locations with a caseload of approximately 105 patients. Key achievements over the years include- Getting trained as an options educator by Joanna Neumann and continuing to provide options education as part of my role to this day (Active collaboration with Kaiser RWC and Kaiser SSF)- Developing and maintaining strong collaborative working relationship with both Kaiser Redwood City and Kaiser South San Francisco- Getting recruited to and helping develop trainings for the home social worker collaborative. trainings I helped develop presentations for included Role of the Social Worker in Pre-Dialysis Home Visits and PHQ9 Implementation Feedback- Helping plan the first post-covid in person Social Work Department all-day gathering at Filoli in 2022
