Assist older adults and their families to cope with the challenges of aging and make positive, informed decisions and plans across the health care continuum•Communicate with physicians, care agencies, caregivers, and other support services on behalf of client and/or family•Research, vet and compare senior community/care options for needed relocation.•Research community resources such as caregiving agencies, senior services, transportationservices, home healthcare, hospice etc. to assist families transition their loved one to the next level of care•Review and recommend Immediate Care/Long Term Care Insurance Policies•Review Veteran benefits that maybe available to client•Offer \"crisis intervention\" when it is needed, helping clients navigate through emergency departments and hospitalizations, rehabilitation stays, and ensure that adequate care is provided to the client as they transition to the next level of care•Assist clients in attaining their maximum functional potential in the safest living environment- provide home safety assessments as needed
