Experience with Case Management while participating in weekly Inter-Disciplinary Group meetings with clinical teams to collaborate patient care/family & caregiver support. Additional support provided to patients, families, and care providers at the hospital in the areas of oncology, heart care, intensive care, emergency department, behavioral health, surgical, birthing, and NICU.
