Management: Typical responsibilities include identifying home care needs and resources available and coordinates assessments, development of care plans and delivery; directs specialized home care programs such as health maintenance, senior companion, and homemaker; develops goals and evaluates programs for efficiency and cost-effectiveness through reviews, audits, cost allocations, and activity reports; develops policies and procedures to ensure compliance with federal and state laws and regulations; develops and administers budget based on program activities, expenditures, and revenue generated; prepares reports including supplying critical information for completion of insurance cost reports; negotiates agreements, contracts, and managed care rates; works with grants; oversees screening and insures competencies of applicants; coordinates with clinical RN staff including orientation and continuing in-service training; develops and implements quality assurance programs. Program development and consultation: Typical responsibilities include consulting with physicians and other health care providers to assess community home care service needs and develops and implements services to meet identified needs; uses statistical reports for planning and determining program direction; analyzes trends in health care to initiate change and to meet legal requirements; consults with health care providers, educational institutions, and consumers regarding services and program operations; promotes public awareness of home care program; participates in panels and commissions; represents home care district at meetings, seminars, conferences, local chamber of commerce, and various organizations and collaboratives. Knowledgeable, trained, and experienced in management practices; home care programs, operations and community resources, health delivery systems including health education for patient centered medical home, OSHA, HIPAA; etcetera; Licensed clinician.
