Collaborate daily with Home Care Agencies, Providers, Members contracted with the insurance company, and Internal Departments to streamline and ensure seamless communication and resolve complex care and service-related issues•Manage high-volume cases of interruption and resumption of care, ensuring smooth discharge transitions for patients from hospitals to homes or nursing homes, while maintaining qualityand continuity of care. •Address Durable Medical Equipment (DME), home care agency scheduling concerns as well as vendor, and Public Partnerships (PPL) issues, providing Authorization numbers and a timely resolution ensuring optimal service to all of the members.
