As the Senior Liaison and Care Transition Coordinator for St. Croix Hospice in metro Detroit, I lead a dedicated team of CTC’s to ensure patients and families experience a seamless transition into hospice care while building strong community partnerships. My role is deeply rooted in fostering relationships with referral sources, such as hospitals, physicians, skilled nursing facilities, and assisted living communities, across the metro Detroit area. I actively engage with healthcare providers, educating them about St. Croix Hospice’s compassionate, holistic approach to end-of-life care, which helps drive referrals and strengthens our network. By maintaining these connections, I ensure our services are top-of-mind for doctors and facilities when patients need hospice support. Beyond outreach, I guide patients, families, and caregivers through the often-emotional process of entering hospice, offering clear, empathetic explanations about our services, including how we address physical, emotional, and spiritual needs. I help them navigate complex decisions, from understanding Medicare and Medicaid coverage to addressing advance directives like DPOA and DNR paperwork. Coordinating with my team, I oversee the intake process, working closely with RN Case Managers, social workers, and other interdisciplinary team members to develop personalized care plans and arrange essential services like medical equipment, medications, or respite care. As a leader, I mentor my team of three Care Transition Coordinators, providing training, shadowing opportunities, and guidance on territory management to meet sales and marketing goals. Above all, I advocate for patients and families, ensuring a smooth transition into hospice care by addressing needs like Medicaid applications or private duty care arrangements. My work is about delivering dignity, comfort, and expert care, and with my team, I’m proud to uphold St. Croix Hospice’s reputation as a trusted provider in Southeastern Michigan
