Proactively contribute to Eskenazi’s mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County. Coordinate outpatient CGM program• Evaluate the nutritional status and needs of patients and develop care plans following the established standards of care and practice guidelines.• Provide nutrition counseling to patients and their families that is age-specific, cultural, religious, and ethnic sensitive for chronic disease management, disease prevention, and wellness.• Consult with other health care professionals and members of the interdisciplinary team to manage patient care across the age continuum; implements nutrition intervention(s) including communicating the nutrition care plan.• Provide appropriate monitoring and follow-up in accordance with the patient’s treatment goals, referring patients for community or home health services as appropriate.• Document patient evaluations, plans of care, counseling, monitoring, and follow-ups in the patient’s medical record or electronic medical record (EMR).• Participate in clinic and community health fairs, public relations through various media venues, as well as nutritional programs.• Participate in the planning and development of outpatient nutrition-related policies and procedures, continuous quality improvement activities for nutrition services, and patient education materials, clinical forms, and protocols.
