    HCPC works to significantly improve health equity, health and overall quality of life for residents of the greater Passaic County area by specifically addressing social determinants of health through a dedicated, thriving and sustainable community coalition and regional health hub. As one of four legislatively designated New Jersey Regional Health Hubs, HCPC provides strategic partnership to the State, leads integrated data analytics to identify health disparities, and unifies community resources by bridging clinical and non-clinical providers. Our Community Action Board is 60+ members strong, representing a wide range of organizations serving the people of Passaic County, including CBOs, public agencies, social service providers, healthcare providers, places of worship, housing agencies, educational institutions and many more. CAB members collaborate and take action on pressing community issues including COVID/public health, opioids/addiction, women\'s health, violence intervention, housing, seniors\' health, and diabetes.(*I transitioned to part-time work in 2021 to spend more time with my family.)
