As a Lead Care Manager in Enhanced Care Management (ECM) and Social Determinants of Health (SDoH), I play a critical role in coordinating and overseeing care for patients with complex health and social needs. My primary focus is to improve health outcomes and quality of life by addressing both medical and non-medical factors that impact patients daily.
