With 13 years of public health experience, my work has been dedicated to improving health outcomes for at-risk populations. In my current role at Constellation Quality Health, I serve as Program Manager and Principal Investigator, leading a five-year CDC-funded initiative to address health inequities in hypertension, cardiovascular disease, hyperlipidemia, and stroke across four states. This initiative focuses on advancing cardiovascular health through social determinants of health, systemic policy changes, and strengthening community-clinical linkages to foster a collaborative network of care. My expertise spans disease management, chronic care, and health education, and I have a proven record of managing community health programs to reduce disparities. I am committed to driving impactful health outcomes through innovative strategies, strong partnerships, and advocacy for equitable healthcare systems that serve diverse and vulnerable populations.

1. Program Manager Principal Investigator

   Oversees a Centers for Disease Control and Prevention (CDC) innovative five year award/cooperative agreement/The Innovative Cardiovascular Health Program aimed at reducing health disparities in hypertension, cardiovascular health, hyperlipidemia, and stroke among minority populations in North Carolina, South Carolina, Georgia, and Florida. The cooperative has four program lines that addresses social drivers of health and advocates for systematic health policy changes. The program provides healthcare providers with enhanced resources to improve health outcomes, creates community-clinical linkages to build a community of practice, enables community-based organizations to use a bi-directional referral platform to address social drivers of health, establishes a community of care hub focused on evidence based community interventions, and ensures equitable access to resources for patients/community members to improve their health and well-being. Additionally, directs a team of public health and subject matter experts and oversees an advisory board composed of leading healthcare and public health professionals.
