I am part of Douglas County\'s Homeless Engagement, Assistance, and Resource Team (HEART) which is a co-responder team that pairs Navigators with local law enforcement to respond to community calls regarding homelessness and provides support and resources to people experiencing homelessness. As part of this co-responder team, I help to strengthen Douglas County\'s support for those experiencing homelessness in our community.As a HEART Navigator, I am a subject matter expert with experience in behavioral and mental health and case management who proactively and directly interacts with those experiencing homelessness in a compassionate way. I gather information on an individual\'s needs, assess vulnerability, provide wrap-around case management, and make referrals to appropriate community services.
