Primarily responsible for ensuring adherence to high fidelity wraparound model of care coordination, ensuring children, youth and family/caregiver centered, high quality services, positive outcomes, and satisfactory business results for the program serving children and families/caregivers of a variety of socioeconomic, racial, ethnic, and cultural backgrounds.* Provide supervision for 8 direct report care coordination team members scheduled at least biweekly.* Provide support and corrective feedback to team members to promote positive outcomes.* Responsible for adhering to hiring practices and policies and procedures related to UMCH Family Services personal policy, as well as UMCH Family Services policies and procedures.* Conduct group/team meetings weekly and biweekly.* Develop and participate in successfully delivering identified trainings to assist in the development of employees.* Complete and maintain high fidelity wraparound training compliance provided by an independent validation entity recognized by ODM.* Review, provide feedback, and approve all required documentation to ensure completion of all required care plans, initial assessments and CANS, outcome reports and pertinent documentation are in accordance with OAC rule 51•••••32 OhioRISE Care Coordination.* Ensures coordination and adherence of on-call rotation.* Collaborate with all relevant systems and key participants within systems pertaining to children or youth and family/caregiver being served to ensure the highest quality of services are being provided with focus on ensuring family voice and choice.* Ability to asses needs in crisis situations and inform response.
