Work with and along side older adult(s), and when appropriate family members and/or responsible parties, to create a comprehensive care plan that addresses the current needs of an individual with recommendations that will support, and promote safety and quality of life. Always striviing to incorporate the wants and desires of the client and communicating those elements into the care plan when possible. The care plans are \"client Centered\" written and are revised as changes take place to promote quality of life and safety. Often we give voice to the individual client when they feel they have no say in how they wish to be cared for and the case manager advocates for those choices.
