Senior member of the SCN management team, leading HEALI’s partnerships with CBO and healthcare provider organizations. Responsible for building and optimizing a strategy for Social Care Network (SCN) and healthcare stakeholder collaborations. Primary liaison between HWCLI and SCN partner Manage communication to CBOs and healthcare providers that are participating as contracted providers on the provider network or seeking to join or otherwise coordinate with the network. Build and maintain strategic and contracted partnerships with CBOs, federally qualified health centers (FQHCs), primary care practices, IPAs, behavioral health providers, and other healthcare providers. Develop and execute strong relationships with primary care providers to integrate healthcare and social care for the optimization of preventative care. Ensure the SCN complies with New York State Department of Health (NYSDOH) network adequacy requirements through oversight of routine network assessment process. Identify opportunities for SCN provider network growth and outreach and advise leadership on strategies to act on these opportunities Set and achieve metrics for CBO and healthcare provider engagement, contracting and contract performance. Oversee SCN provider onboarding activities and educational programs, with a particular focus on member/client screening, referral, and case closure. Manage the SCN CBO capacity building program to equip CBOs with resources to provide services on the network. Monitor network provider readiness to receive and process payments for services rendered to members/clients. Oversee the collection and management of all network provider on-boarding forms and other documents relating to the status of network providers in implementing required components of their contracts with the SCN, including documents related to the CBO capacity building program. Supervise all Network Development Staff.
