Collaborate with community strategic partners in a collective impact manner to reduce health disparities and address social determinants of health by making healthy opportunities accessible, affordable, and default for all residents. Organize and/or lead community collaborations with a broad range of stakeholders including government, non-profit, corporate, neighborhood, and community leaders to build and implement plans driving toward opportunities for impact to improve health equity for all. Execute patient engagement strategies and community programs and events to address obesity, diabetes, cholesterol, mental health, hypertension, heart valve disease, heart disease, stroke and social determinants of health. Build and implement a cross-functional hypertension and cholesterol control strategy for Orange County and Coachella Valley to include strategies to promote and implement Target: BP and self-monitoring programs including Check. Change. as well as Check. Change Control. Cholesterol. Identify and recruit corporate CEOs, civic leaders, and philanthropic volunteers who will serve in leadership roles on the board and committees to advance our mission and raise critical funds. Drive organizational change through employers to build a culture of health by adopting and completing AHA’s Workplace Health Achievement Index. Engage multicultural communities through events and programs which support the AHA mission, within priority zip codes/vulnerable populations. Recruit, train and mentor college-level students in the Legacy Leader Internship program to help drive community transformation and healthy living opportunities through health impact and engagement.
