Investigates positive laboratory tests of infectious diseases (HIV, syphilis) within the Northeast Ohio region as guided by the Ohio Infectious Disease Control Manual-Conducts community-based outreach within the region to notify and interview individuals on a case load who are at risk or diagnosed with an infectious disease (HIV, syphilis)-Makes primary and partner referrals for testing and treatment-Follows up with individuals diagnosed with syphilis to ensure treatment is completed and follow-up testing is done-Contacts healthcare providers as needed to gather additional information-Provides referral, education, and other risk reduction information about HIV/syphilis/STI prevention and linkage to care services
