The Department of Family Medicine at Cook County Health and Hospitals System (CCHHS) has a long tradition of service and education in the safety net and has a pivotal role in reducing health disparities and optimizing public resources. CCHHS proactively launched CountyCare, a Medicaid Managed Health plan, in 2013 in anticipation of the ACA roll out. Family Medicine is positioned to help lead primary care practice transformation as part of an integrated health care system that includes the Stroger academic hospital/medical center, two community hospital campuses. more than a dozen community health centers, Cook County Health Department, heath centers for Cook County Jail and Juvenile Detention Center, the Core Center for HIV, etc. Our large and growing Family Medicine Department is made up of some amazing, caring and dedicated family doctors, a well established residency and faculty development fellowship program. Our team provides full scope family medicine in patient centered medical homes, inpatient units and on labor and delivery. I am honored and privileged to now be a part of the CCHHS Family Medicine team.
