   I help provide access to quality preventative care to seniors and older adults.I am often the first voice a patient hears representing Cadence. I focus on building rapport, trust, and credibility while actively listening to understand patient needs and clearly communicating the value of our programs.In just three months, I have enrolled 482 patients into our advanced primary care and remote patient monitoring programs.That is impact.Four hundred eighty-two people who now have support from Cadence Care Teams, addressing things like social determinants of health between visits and ensuring the whole person is cared for.
