Patient-centered and trauma-informed health coaching in clinical and non-clinical sites in collaboration with medical providers- Conduct and monitor weekly group coaching to 10+ patients and 1:1 coaching at multiple sites - Support patient with desired realistic behavioral change by goal-setting (SMART Goals), motivational interviewing, and unconditional positive reinforcement techniques - Facilitate and lead virtual wellness programs through skilled-practice and group discussion- Develop evidence-based materials and lesson plans on stress reduction techniques, sustainable nutrition habits, and active living - Identify health risk factors and encourage chronic disease prevention methods
