Conduct comprehensive transition plan compliance checks- Facilitate one-to-one feedback and corrective action meetings- Provide guidance on student-centered decision-making toward post-secondary outcomes- Identify and create business alliances to support training and employment opportunities- Advocate for the inclusion of neurodiverse students and adults- Participate in multi-disciplinary meetings to guide the post-secondary transition process- Contribute directly to the development of synchronous/asynchronous learning opportunities.
