In 2019 the City of Hyattsville, MD, adopted a community-driven action plan to become a more livable municipality based on the World Health Organization\'s Age-Friendly Cities framework. In my role I lead this work.Working with residents and volunteers, local, state and national organizations, area businesses, and other stakeholders, we are collectively identifying and addressing barriers that impact the well-being and inclusion of older residents and community members with disabilities. Our action plan and work, focused on six interconnected domains, recognize that addressing social determinants of health (SDOH) is integral to enhancing people\'s health, well-being, and quality of life.
