About Marc Rosen
Healthcare organizations must address social risk factors to be successful in a fee-for-value environment. I\'ve spent the past 15 years implementing targeted population health interventions that link population health management objectives with community-based solutions to improve community health. During this time, I\'ve remained committed to demonstrating the value that community-based solutions can deliver as part of an expanded care continuum. These experiences inform my expertise in bridging the cultural, knowledge, and strategy gaps between the healthcare and community-based sectors to evolve how we address treating social needs in clinical and community I have delivered regular thought leadership via presentations, panels, and facilitated meetings to community-based organization and healthcare leaders on how to leverage the role of social service providers in a value-based care environment. I have implemented an enterprise-wide strategic framework used to measure and monitor the performance of social care coordination networks across more than 40 states. I\'ve consistently been a champion of diversity, inclusion, and equity throughout my career, and served as the Chair of Y-USA\'s Diversity Opportunity Team for two years. Want to talk?
