   As a transformational leader, I designed a strategy and 4 phase-plan to transform the business model from a fee for service model to a per-patient-per-month (PMPM) model. The plan used the innovator’s method and existing resources to accelerate growth while testing hypotheses with pilots and customer feedback. The first phase, based on demand generation, differentiated the overall portfolio based on industry trends and customer needs. I led Manufacturing and the Durable Medical Equipment (DME) organization to align on an offering to develop a service to meet customer needs. I launched the disease management services for chronic respiratory patients, CARE Connex, to create a unique differentiation in a commodity space. The second and third phases monetized the patient data. The fourth phase consolidated the results from the first three phases to further define and enhance the services provided by the DME and consolidate a PMPM model directly with payers. Coached and mentored the Connected Outcomes Services team. Delivered results including, new outcomes services, sales tools, and customer segmentation. Led Singapore engineering team to accelerate connectivity development by creating the first connectivity software roadmap within the division. Connectivity included: a customer portal and app, EMR/HL7 communication and the hardware required to transmit the data.
