In my clinical Registered Dietitian role, I evaluate and enhance patients\' nutritional health status in the acute care setting. I screen individuals for nutrition risk by assessing MST scores, skin triggers, mechanical ventilation status, and admission diagnosis. Following this initial screening, I conduct comprehensive nutrition assessments and document my findings using the Epic charting system in alignment with the Nutrition Care Process. This involves obtaining diet and weight history, Nutritional-Focused Physical Exam (NFPE), analysis of nutritionally significant labs and medications, calculation of estimated needs, nutrient analysis using CBORD and Room Service Choice, and identification of malnutrition using ASPEN\'s criteria. Guided by these assessments, I develop personalized nutrition care plans, implementing tailored interventions such as diet order adjustments, initiating oral nutrition supplements, vitamin and mineral supplementation, adding meal protocols, and providing recommendations for enteral and/or parenteral nutrition support when indicated. Furthermore, I provide patients with comprehensive nutrition education using materials from the Nutrition Care Manual, covering various dietary needs such as CHO Controlled, Heart Healthy, Renal, Low Fiber, Bariatric, and more. Additionally, I attend interdisciplinary rounds on both general floors and in the ICU, in which I collaborate in the development and execution of effective patient care plans.
