I am part of the Older Adult Services Team of this non-profit Federally Qualified Health Center (FQHC), which is the largest of its kind in IL and the sixth largest FQHC in the nation. Serving patients in and around Chicagoland, ACCESS provides services across the lifespan, ranging from Prenatal Care to our Older Adult Population - regardless of citizenship or medical coverage status.Using a comprehensive, integrated approach to our medical practices, ACCESS brings together a robust array of services and resources to support our patients’ overall health and wellness. For more than 30 years, ACCESS has been on the frontlines of community-based health care, working to address racial and ethnic disparities and improve health equity in some of the most vulnerable communities across the Chicagoland area.
