Create Individualized Plan:Our RCPs create a home care plan outlining education and safety for patients and their caregivers. Every case is unique, and every plan matches each patients individual needs.Training and Assistance:The RCPs train families and caregivers to operate the equipment for the patient, answering questions and facilitating a safe transition home.Continued Evaluation:We assist the family and caregivers to make sure there has been a smooth transition to home and if there is anything we can do to help in the next phase. Monthly Follow-Ups:The patient receives monthly supply replenishment and clinical visits by an RCP. We’re here for the long term!
