Develop and execute the vision for providing key direction to health equity-driven performance strategy and population health interventions within Ascension’s CHM portfolio. As part of the Leadership team, I partner with Clinical Leadership, Operations and Program leads across multi-state clinical sites to develop and implement value-based programs, monitor performance metrics and accountability, and improve performance measures as needed. Responsible for supporting the VP in leading program implementation for value-based care, population health, specialty value-based care solutions and CHM reimbursement innovation.
