Praxis has roughly 100 primary care providers and patient lives- Value based care manager. VBC contract include P4P/quality, shared savings and full risk- Care management manager: Developing and evaluating Praxis approach to care management for our high risk patients- Manager of risk adjustment coding: Developing, managing and evaluating programs to improve our risk adjustment coding. I manage a team of risk adjustment coder to assist in these efforts- Utilization and Transitional Care management manager: educate, train, evaluate and manage our efforts around the transitions of care process, high utilizers and ED follow ups- Manager of our quality improvement/ HEDIS department: develop, manage and evaluate programs and project to improve our performance on key HEDIS measures. For example optimizing our EHR, enhancing our supplemental data files and improving workflows related to quality measures. I manage a team of HEDIS coordinator to assist in these efforts.
