Responsible for facilitating improved overall quality, education, and completeness and accuracy of medical record documentation as well as promoting appropriate clinical documentation through extensive interaction with providers and HIM coding staff. Serve as technical expert in health record content and documentation requirements. Perform reviews of the health record documentation; develop criteria, collecting data, graphing and analyzing results, creating reports and communicating in writing and/or in person to appropriate leadership and groups. Obtain corrective action plans from responsible clinical services directors and recommend improvements or changes in documentation as necessary. Adhere to established policy and medical legal requirements. Responsible for the development and implementation of active training/education programs (i.e, seminars, workshops, short courses, informational briefings, and conferences) for all providers to ensure the CDIS program objectives are met.• Adheres to accepted VHA coding practices• Monitors ever changing regulatory and policy requirements affecting coded information• Assists facility staff with documentation requirements, provides technical support in areas of regulations and policy and coding reimbursement.• Searches patient health record to find documentation justifying code assignment• Facilitates improved overall quality, completeness, and accuracy of health record documentation• Collaboratively works with the professional clinical staff and provides support and education•Participates in CDI Huddles and offer information on documentation, coding rules and VERA issues• Acts as liaison between the coding department and the clinical documentation specialist to reconcile discrepancies in DRG assignment.• Acts as CDI representative on PSI and Mortality review committeeCollaborates with CDI RN Advisors when specified educational needs are identified
