Admissions of hospice patients: completing an initial and comprehensive assessment of patient and family to determine hospice needs and confirming hospice appropriateness. Providing a complete physical assessment and history of patient\'s current and previous illnesses. Providing professional nursing care by utilizing all elements of nursing process. Assessing and evaluating patient’s status by: o Writing and initializing plan of care. o Regularly re-evaluating patient and family/caregiver needs. o Revising the plan of care as necessary. Initiating the plan of care and making necessary revisions as patient status and needs change. Using health assessment data to determine nursing diagnosis. Developing a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions, and consciously including the patient and the family in the planning process. Administering medications and treatments as prescribed by the physician in the plan of care. Educating the patient, family, and caregivers in meeting nursing and related needs. Acting as Case Manager and assuming responsibility to coordinate patient care for assigned caseload. After hours triage nurse.
