Intake Assessments-Evaluate homeless households using community-wide information-Establish a base of ongoing assessment locations (social service programs, hospitals, shelters, etc.)-Conduct assessments using the VI-SPDAT evidence-based screening tool-Provide relevant, targeted resource referralsHousing Placement-Assist households with developing and implementing housing stabilization plans-Advocate on the client\'s behalf with landlords, property managers, etc-Assist participants in identifying and securing stable housing -Apply knowledge of residential lease contracts to educate clients on their rights and responsibilities-Coordinate and conduct habitability inspections on relevant properties -Process requests for financial assistance i.e, rent depositsProgram Development-Create and cultivate relationships within the homeless services providers to establish best practices among housing navigation-Create collaborative relationships with other Continuum of Care service providers and community organizations to provide families with additional supportive services-Participate in Continuum of Care (CoC) activities and pursue additional educational events to expand knowledge of topics surrounding best practices for housing providers and trending methods consistent with HUD initiatives-Ensure daily that all program activities are consistent with program mission, goals, and anticipated outcomesRecord Management and Reports-Maintain accurate case notes and monthly outcome reports-Maintain client files to include all signed forms, policies, and other housing documentation-Submit monthly, quarterly, annual, and other program performance reports as required-Enter all necessary information into the Homeless Management Information System database in a timely matterAdministrative/Other Duties-Attend weekly case management and general staff meetings-Attend CoC meetings & community-based conferences and trainings
