Initial Patient Triage and Management: This role involves conducting initial assessments for new patients, entering their information into the electronic medical record (EPIC), and scheduling their nephrology visits.• Patient and Caregiver Support: provide education on chronic kidney disease as a health condition and offers ongoing care coordination, connecting patients to necessary resources.• Data Management and Tracking: manage patient data, tracking their enrollment status in the program, and documenting all care activities within the electronic medical record system (EPIC).• Interdisciplinary Care Coordination: work closely with various interdisciplinary teams (internal medicine, nephrology, cardiology, social work, etc.) to ensure patients receive comprehensive care, including managing social and health-related needs.
