Lead set-up, implementation, evaluation and growth of integrated neighbourhood working across SEL. Spearhead the pioneering multi-morbidity model of care programme for the SEL cardio-renal metabolic multiple long-term conditions (mLTCs) population. Develop and scale this person-centred, holistic, integrated model focused on proactive intervention and improved population health outcomes. Key achievements include: • Identified and launched pilot sites across SEL; led four large-scale, in-person workshops with 100+ attendees each to establish integrated neighbourhood teams across all six boroughs, driving both vertical and horizontal system integration.• Directed population risk stratification to enable proactive, preventative care; supported development of a tailored patient care pathway aligned with the needs of individuals with mLTCs with over patients reached to date. • Independently established programme governance, reporting frameworks, communication channels, and a regional community of practice to support a ‘test and learn’ approach to optimising patient care.• Chaired the evaluation working group and designed a two-phase evaluation strategy, combining quantitative (prevalence, health outcomes, hospital admissions) and qualitative (staff/patient experience) measures; produced a comprehensive Phase I report and accessible ‘easy read’ presentation.• Designed and delivered cross-system education and training events for ~500 clinical and non-clinical staff across SEL, complemented by a podcast series developed with clinical education leads.• Authored a “how-to” guide for implementing integrated neighbourhood working to support system-wide spread and adoption.• Present programme insights, lessons learned, and early outcomes at local, regional, and national events to promote best practice and collaborative learning.
