Provide direct clinical social work services and healthcare navigation to patients and their families in relation to social and environmental problems. • Assess the psychosocial needs of patients and their families and develops appropriate interventions/treatment plans to meet identified needs. • Provides direct clinical social work service to patients in relation to emotional problems. • Build and maintain strong partnerships with community organizations, mental health providers, and social service agencies to identify and coordinate support programs for families.• Provide patient advocacy for appropriate accommodations/modifications based on the patients’ educational needs within the school. • Connect families with appropriate resources such as counseling, tutoring, health services, housing support, and enrichment programs.• Maintain accurate and confidential records of patient interactions, referrals, and progress.• Monitor and follow up on support plans to ensure services are effective and coordinated.
