    The Case Management Coordinator will have primary responsibility for ensuring coordinated, tailored case management for program participants of BASIC ( Beyond Services to Afghan Survivors Impacted by Combat.)They will carry a partial panel of patients for navigation/ care coordination. They will supervise 4 bilingual Patient Navigators, confirming that regularly scheduled check-ins occur with patients and that patients are appropriately and efficiently referred to partnering agencies/community-based resources as needed.As indicated, the position will also lead coordination with IINE-L services and support related project activities.Provides case management inclusive of social assistance, including medical and behavioral health coordination, to patients impacted by combat from Afghanistan. The BASIC delivers culturally tailored, integrated, trauma-informed, and evidence-based primary care and behavioral health care services; workforce-related, legal, and other supports; and community outreach to promote well-being, self-sufficiency, and community engagement among our eligible Afghan population.
