Mental Health Outreach Counsellor - Primary Care Outreach Program @Durham Region (The Regional Municipality of Durham)

Work in partnership with an advanced care paramedic providing mobile outreach health and mental health services to the unsheltered and at risk of being unsheltered community- Provide outreach mental health services, including biopsychosocial assessment, psychotherapy, and navigation to resources to address identified needs, including housing, food insecurity, income, medical care, and legal services and document interactions- Collaborate with community partners, develop and deliver seminars, and participate on committees to support the unsheltered.
