The Complex Care Management Program (CCMP) works with all Michigan Medicine Care team members including Primary Care, Specialty Care, Emergency, Inpatient and Community providers to coordinate comprehensive services that promote the total well-being of vulnerable persons while working toward reducing avoidable healthcare costs.Goals of Complex Care Management:• Address patient barriers to care (internal and external) that prevent and/or impact their ability to access safe and necessary healthcare services. • Decrease avoidable health system utilization of ED/Inpatient stays. • Improve appointment attendance, medication adherence and engagement with PCP/Specialty site. • Build care teams within MM and with community partners to sustain patient needs throughout time.
