About Kristofer Smith

For the past 15 years I have been a national physician leader in population health and value-based care. My leadership roles have focused on delivering results that matter both clinically and financially through the build of high performing management teams, products, and provider groups.My current role as President and CMO at Ruby Health allows me to take the lessons learned from a career helping the patients who need it most. We are building a platform to enable home health agencies succeed in value based care. We will help patients stay at home through care management and a virtual medical group.At Northwell Health, I built the organization\'s care management capabilities. As an executive director, medical director, SVP, and health plan CMO, I grew the new entity to over 400 FTEs, with annual revenue in excess of $40M. Our team led the nation\'s highest performing Bundled Payment for Care Improvement and Independence at Home CMMI demonstrations. We nurtured New York State\'s highest performing Health Home-a care coordination program for high-risk, community dwelling Medicaid patients. At naviHealth, as President and owner of the P&L, we built a tech enabled medical group and shared services platform to bring care to high risk patients in partnership with BCBSMI. At Prospero I was responsible for training, network, product, and eventually the P&L of the entire field staff across 26 states. Strong engagement, outcomes, and revenue performance enabled a strong exit with the entity being acquired by Optum. At Optum, I was the CMO for the combined Landmark and Prospero business and the executive sponsor for the unification of these two entities into a single platform. I led the efforts to optimize cost and model design, achieving higher performance at a lower PMPM cost. My team was also responsible for product development and clinical innovation. At Optum at Home, in addition to quality, documentation, on-call, medical group and clinical subspecialty operations, I was responsible for affordability performance across the 1.1M DSNP and 0.5M MA lives under risk.
