ABOUT KRISTIN M. SWEET

I have been privileged to spend my career working with the aging population and adults with chronic physical and mental health conditions. My work, especially providing counseling and support with life transitions, has been both challenging and deeply rewarding. I love seeing clients and families grow to become their own best advocates. My experience in medical social work and behavioral health illustrates my ability to successfully coordinate with individual clients and complex systems. I also understand the importance of linking clients to community programs and resources as part of a well-rounded plan of care.
