Participated in multiple projects with the UM team and Nor Cal Region Leadership to ensure the continued success of high H-CAHPS scores among members. • Group member comprised of UM PCCs and inpatient MSW to establish a more detailed work-flow amongst the PCCs and MSW to ensure optimal collaboration and care for hospitalized KP members. • Monitor the progression of the discharge plan and management of resources through multidisciplinary rounds and collaborations with families, physicians, and staff. • Develop, evaluate, and coordinate a comprehensive discharge plan and establish continuity of care in conjunction with the patient/family, physician, nursing, social services, and other health care providers and agencies. • Perform daily admission and concurrent utilization reviews via InterQual to determine appropriate levels of care.
