AFFINITY CARE HOSPICE OF MANATEE COUNTY IS NOW MEDICARE CERTIFIED! On August 24. 2023 we received a deficiency free Medicare survey and are now CHAP accredited. Our hospice team will be bringing the same high level of clinical excellence and comprehensive psychosocial spiritual support to patients and residents of Manatee County as we are known for in Sarasota County.Responsible for developing and maintaining referral relationships through marketing efforts with assisted living facilities, memory care facilities, skilled nursing facilities, physicians, nurses, support staff and home care professionals in the community.I influence community healthcare professionals through hospice education and networking, helping them identify and refer patients and their families. I provide education to physicians, hospitals, facilities and families on the many benefits of the Hospice Medicare Benefit and the hospice levels of care.
