Negotiating risk-reduction plans and making referrals to hospitals and case management. Also responsible for providing Tuberculosis Skin Tests/HIV Rapid Testing/Confirmatory testing at drug treatment facilities/Fort Bend County jail system of Houston for adult and youth clients. This includes extensive documentation of statistical information within the (CPCDMS) Centralized Patient Care Data Management System and BIHIPS/RECON Systems, while providing support to individuals in making behavioral changes to reduce their risk of acquiring or transmitting HIV/STD’S and HCV.
