Provide home and community-based early childhood special education services to families of children between the ages of birth and three with developmental delays or diagnosed conditions.• Use the coaching model to encourage families to implement strategies within their daily routines in order to further their child’s development.• Conduct initial evaluations to determine eligibility for the Birth to Three system. Sensitively convey evaluation results to families and write detailed evaluation reports.• Service coordinate for many families, to include helping the families create Individualized Family Service Plans and review them regularly, updating assessments and evaluations, writing assessment and evaluation reports as necessary, and gathering appropriate resources for each family.• Empower families to navigate the special education system by preparing them for the transition to preschool and accompanying them to Planning and Placement Team Meetings- Provide ongoing support to our team of teachers, social workers, and behavior analysts to include answering procedural and clinical questions, as well as facilitating monthly discipline specific support meetings.
