Responsible for overseeing all aspects of case management and population health for inpatient, ambulatory, and ACO Medicare, Medicare Advantage and Commercial lives. Franciscan Health has 15 campuses throughout Indiana and Illinois. We have approximately lives covered in an ACO model and are also participating in Medicare joint bundle. Healthcare is constantly changing and we are blessed to be involved during the expansion of population health.
