ACO DirectorOversee ACO overall operations to ensure meets the goals for quality, performance metrics, care coordination and ensure to follow compliance regulations, aim to quality of patient care and cost effectivenessQuality Team Manager:Oversee and coach the team performance and completion of our programs within the established deadline and percentages. Identify areas to grow and improve as a department or per clinic level providing the necessary training and education. Quality Training Coordinator:Coordinate educational trainings based on trends found in the documentation following CMS and coding guidelines. Coordinate the overall preparation of educational events to providers such as Bootcamp and make sure they have the necessary materials for the event. Onboard new team members and/or providers.Quality Analyst:Identify training needs, prepare summary reports and conduct coaching as appropriate for MD, PA, NP, DO, and other staff members to improve quality of the documentation to reflect member\'s health data. Identify suspect medical conditions and query the provider to evaluate the patient for potential coexisting conditions.Ensure compliance with federal and state laws related to coding and documentation guidelines.Ensure diagnosis codes for each major and chronic conditions have been captured and supported by documentation guidelines for coding and reporting to CMS.Communicate and coordinate concurrent chart review with physicians and share feedback related to review.
