CIN Manager serves as the primary conduit between physicians and clinicians, their practices, theCIN, and the hospitals. The CIN Manager will encourage physician engagement and retention with theCIN through open dialogue and in-person visits with both employed and independent affiliated physicianpractices. The CIN Manager will support CIN medical leadership and the THMA CIN Director ofPopulation Health in the implementation of individual performance improvement programs designed toimprove quality and efficiency of care delivery with the overall goal of achieving quality care, whilelowering health care costs.Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Valuesin behaviors, practices, and decisions.Promotes ACO/CIN participation to independent physicians to solidify affiliation.Obtains participating physician signatures on new and renewal agreements from individualparticipating physician practices.Orients and onboards both MGPS and independent affiliated CIN physicians to the basic mechanismof value-based care and alternative payment models. In collaboration with CIN data/analytics and care coordination, identifies areas for performanceimprovement, both utilization and quality, and educates physicians and their office staff accordingly.Schedules meetings with physicians and their office staff to review monthly performance scorecard,care gap lists, high-risk patient lists, and recent ER/acute admission activity with their attributedpatients, and other pertinent topics. Performs/assists with the annual governmental and commercial payor program quality audits.Coordinates regular All-CIN Physician Meetings and other ad hoc WebEx presentations. Fields andresolves questions, concerns, and other issues physician practices may have specific to value-basedand fee-for-service co-branded arrangements.Assists with CIN roster maintenance and updating of information with intel from the field.
