I am passionate about providing optimal customer-centered care through the development and implementation of a personalized approach to healthcare services. The depth and breadth of my professional experience has provided me greater insight into the healthcare industry and has inspired me to apply my passion for customer and provider advocacy to new and innovative approaches for the improvement of healthcare delivery systems. A few of the accomplishments I am most proud of include: June 2018 – I was instrumental in bringing Cigna in as the lead sponsor for the Connecticut Better Health Conference. This opened opportunities for Cigna to promote their mission to over 500 healthcare providers and community. February 2016 - As the Clinical Program Senior Advisor at Cigna, I work cross-functionally with my matrix partners to retain and obtain new business in Connecticut. June 2014-February 2016 - As the Director of Care Coordination for CSMS-IPA, I established clinical programs to support independent physicians in achieving the triple aim across multiple value-based relationships. February, 2013 - Implemented HeartTalk within CHH and the Northwest Community. HeartTalk is an evidenced-based educational program for patients diagnosed with Congestive Heart Failure (CHF). The program was developed by Qualidigm and other community partners funded by CMS. This program was designed to help educate and support patients in living well with CHF, promoting self-management. I developed and facilitated educational in-services for patients,staff and community healthcare professionals. July, 2012 - Spearheaded and served as the lead facilitator for the Northwest Transitions in Care Community (NWTCC). A program designed to identify and create solutions to gaps in care throughout the care continuum. Worked with over 30 health care providers including visiting nurse agencies, skilled nursing facilities, medical supply companies, assisted living facilities, Qualidigm (QIO), and other community care providers. The team met monthly to discuss and review cases of readmissions to formulate plans for a collective response to improve patient care. January, 2010 - Served as the Care Coordinator for a new venture aimed to reduce the number of hospital readmissions. Through the development and implementation of a coordinated discharge plan and post discharge follow up the program resulted in a 50% reduction of readmissions.
